2. Complete the top portion in full

3. Initial parts 1 through 9 to indicate that you have read and understand the conditions of this permit

4. Sign the permit on the bottom

5. Keep a copy for your records

6. Return the original copy to:

Central Lyon County Fire Protection District

You may fax your signed burn permit to the District at 775-246-6204.
Please include your fax number so we can return a signed copy of your burn permit.
