1. Print document
2. Complete the top portion in full
3. Initial parts 1 through 9 to indicate that you have read and understand the conditions
4. Sign the permit on the bottom
5. Keep a copy for your records
6. Return the original copy to:

Dayton, NV 89403

You may fax your signed burn permit to the District at 775-246-6204.
Please include your fax number so we can return a signed copy of your burn permit.

BURN PERMITS EXPIRE MAY 20, 2012
OR AT THE DISCRETION OF THE FIRE CHIEF

Per Nevada Administrative Code NCA 477, Any person who violates the terms or conditions of this Permit shall be guilty of a misdemeanor and may be cited as such. The expense of responses to and suppression of any emergency incident that is the result of a violation of this Permit may be charged to the Responsible Party whose violation caused the response of equipment and personnel.
